This form is to be used for lodging a formal complaint or an
internal appeal regarding the College, its staff, students,
education agents, products, services, or decisions.
In accordance with the Policy, the prescribed procedure must be followed.
Prior to lodging this form, you are required to have a discussion with the
person who is the subject of the complaint or appeal, in an attempt to resolve
the matter informally.
You should provide all supporting documents or witness information relevant to
your complaint or appeal at the time of submission. Please note that any
complaint or appeal submitted without supporting evidence may not be taken
into consideration.
